1- Are you a first-time CSA customer? *

Yes, and it's amazing! Yes, and I'm still getting used to it No, this is not my first season with Our Table No, I've participated in other CSAs before

2- Are you also a Consumer Member of Our Table Cooperative? *

Yes, I love owning a part of my local food system! Not yet, but I will be soon! No, I didn't even know there was a difference No, and I’m not interested

3- How did you first learn about Our Table’s CSA? *

From a great friend Through a totally random web search Through PortlandCSA.org, Localharvest.org, or some other referral site (answer below) I have no idea where I heard about it

4- How is the quality and flavor of the vegetables you received in your shares? *

The best I've ever had Excellent Good for the most part, but you could stand to improve on the_________ (answer below) Terrible, inedible

5- Are the pick-up times and locations convenient for you or do you have suggestions for other times and locations? *

Super convenient I am accustomed to picking up at my current location and time every week I like _________, but it would be great if you could change ____________(answer below) I never remember to come pick up and it's pretty far out of my way

6- Do you feel like the CSA was a good value (price and volume)? *

Yes, it was perfect for my budget and my diet! Yes, I liked getting just a taste of what Our Table grows Not really, I expected more veggies Not at all, I was not satisfied with the shares

7- Have you visited the on-farm grocery at Our Table? *

Yes, I love it and do a lot of my shopping there! Yes, but it’s not somewhere I go frequently to purchase food No, but I am planning to go there soon No, it’s way too far away from where I live

8- Would you be interested in receiving meat, eggs, milk, other groceries and prepared meals along with your vegetable CSA? *

Yes, I’d love to get most of my food from Our Table! Yes, but only if I get to choose what I get every week Maybe, depending on the price and convenience Not at all, I prefer where get those items now

9- Would you purchase a CSA from Our Table again? *

Definitely, I’m already signed up for summer! Yes, but not in the summer because my garden is exploding with produce! Only if the share size or price changes No, it just isn’t right for my needs

10- Anything else about the farm or CSA you'd like us to know? *

I’m hooked on everything Our Table! The _________ is the best! (answer in Comment box below) I'd really like to be more connected with other CSA customers and Co-op members Other (answer in Comment box below)
